Training Waiver

I entirely upon my own initiative and risk and responsibility am training at the Smoothflo Academy. I am at least 18 years of age. If I am under 18 years of age, a parent or guardian will sign on my behalf.

I understand that Brazilian Jiu-Jitsu can cause serious injury or in extreme circumstances even death. I understand that I will be learning to immobilize an opponent by choking them which can lead to unconsciousness or applying leverage techniques to which can cause major trauma to joints such as the shoulder, knee, or elbow.

Like with most combat sports I understand that at some point everyone who trains in Brazilian Jiu-Jitsu long enough will suffer at least a minor injury.

Knowing all of the above, I freely choose to participate in Brazilian Jiu Jitsu classes at the Smoothflo Academy and acknowledge my risk. I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this exercise program. I acknowledge that my enrollment and subsequent participation is purely voluntary and in no way mandated by the Smoothflo Academy.

In consideration of my participation in this program, I hereby release the Smoothflo Academy and its agents from any claims, demands, and causes of action as a result of my voluntary participation and enrollment. I fully understand that I may injure myself as a result of my enrollment and subsequent participation in this program and I hereby release the Smoothflo Academy and its agents from any liability now or in the future for conditions that I may obtain.

BY OPTING-IN WITH MY FULL NAME AND SELECTING THE CONSENT CHECKBOX, I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.
